DERME IMAGE

Terms & Conditions

Welcome to DERME IMAGE. By booking an appointment, accessing our services, or using our website, you agree to the following Terms & Conditions.

1. Scope of Services

DERME IMAGE provides clinical and dermoscopic imaging services, including mole mapping and skin lesion monitoring, intended to assist in the early detection and surveillance of skin lesions and melanoma.

DERME IMAGE does not provide emergency medical care. Mole mapping does not replace a full skin examination or ongoing care by a physician, dermatologist, or other licensed healthcare professional.

2. Medical Disclaimer

The information, reports, and imaging provided by DERME IMAGE are intended to support skin surveillance and monitoring.

• Mole mapping is an adjunctive imaging and monitoring tool and is not a guarantee of skin cancer detection or prevention.
• Not all skin cancers are pigmented or detectable through imaging.
• Certain lesions may evolve between imaging sessions.
• Areas obscured by hair, tattoos, nail polish, clothing, or body positioning may not be fully visualized.
• Patients remain responsible for monitoring their skin and seeking medical attention for concerning or evolving lesions.

Any suspicious findings may require further medical assessment, biopsy, or treatment by a physician or specialist.

3. Referrals & Follow-Up

DERME IMAGE may recommend follow-up with a healthcare professional, dermatologist, plastic surgeon, or other specialist based on imaging findings.

Patients are responsible for arranging and attending recommended follow-up appointments and investigations.

4. Appointments & Payments

By scheduling an appointment, you agree to comply with DERME IMAGE’s payment, cancellation, and missed appointment policies.

Fees for services are private-pay unless otherwise stated. Insurance reimbursement is not guaranteed and depends on the patient’s individual insurance provider and coverage.

5. Results & Reports

All imaging and reports are reviewed by a dermatologist or qualified healthcare professional associated with DERME IMAGE.

Reports and recommendations are based on the clinical and dermoscopic information available at the time of assessment.

DERME IMAGE reserves the right to amend reports if additional clinical or pathology information becomes available.

6. Patient Responsibilities

Patients agree to:

• Provide accurate medical and contact information
• Inform DERME IMAGE of any new or changing lesions
• Attend recommended follow-up appointments
• Follow preparation instructions prior to imaging appointments
• Remove nail polish and avoid makeup or lotions when instructed
• Notify DERME IMAGE if pregnant, medically unstable, or unable to complete imaging procedures

7. Image Ownership & Records

Clinical and dermoscopic images obtained during appointments remain part of the patient’s confidential medical record.

Patients may request copies of records in accordance with applicable privacy legislation and clinic policies. Administrative fees may apply.

Certain export formats may be limited by the imaging system used by DERME IMAGE.

8. Privacy & Confidentiality

DERME IMAGE is committed to protecting personal health information in accordance with applicable privacy legislation in Quebec and Canada.

Please refer to our Privacy Policy for additional details regarding the collection, use, and storage of personal information.
